This is where forms, applications, and permission slips for upcoming events can be printed out at home
(since most forms seem to lose their way upon exiting the building).
This is the basic agreement for renting The Fuse Box for your own purposes. Please be advised that this is the generic form. Once an agreement is made for your event though e-mail, a specified form will be e-mailed to you for your completion. Please mail this form back to The Fuse Box (with proper deposit, if required) 14 days before your event starts.

Those groups riding our bus to Teen Camp in London, Ohio need to present this boarding pass upon entering the bus on Sunday morning, June 23rd, at 4:45am. Please have it signed by a parent or guardian. PLEASE NOTE: All guests 18 and under are required to turn in $40 (on top of their camp fee) in the form of FIVE DOLLAR BILLS to be handed to them at each meal stop.

Location: Kid's Camp in Virginia Beach, VA and Teen Camp in London, OH
Shock Start is for teens who are ready to delve into a deeper understanding of God’s word in their lives. If you're a teen who is interested in knowing the Lord in a deeper way, show this form to your parent or guardian. This small group environment will help teens live out a morally mature Christian life style with one-on-one accountability and biblical training.
Any student utilizing the bus for transportation to and from youth events (including Youth Nights) must have a Transportation Release Form on file.
Notarization is what we use when we need to know that a parent actually signed a form and a student didn't forge it themself. :)
Most Fuse Box forms don't need to be notarized. We only use notarization for camps and semi-risky business such as high ropes courses, paintball, and etc. Most banks, post offices, and insurance companies have a Public Notary (the person who stamps your form). Sometimes it's free at your own bank; other times you may have to pay somewhere around $5.
